🔹️Anaphylaxis(criteria)
درباره آنافیلاکسی تشخیص و شروع سریع اپینفرین IM(بخش خارجی ران) خیلی مهمه. حتی im رو بر sc و iv bolus ارجح دانسته چون راحت تره و عوارضش از iv کمتره. ما سیر آنافیلاکسی رو نمیتونم پیش بینی کنیم.
▪️Diagnosis is made clinically:
The most common signs and symptoms are cutaneous (eg, sudden onset of generalized urticaria, angioedema, flushing, pruritus). However, 10 to 20% of patients have no skin findings.
▪️Danger signs – Rapid progression of symptoms, respiratory distress (eg, stridor, wheezing, dyspnea, increased work of breathing, persistent cough, cyanosis), vomiting, abdominal pain, hypotension, dysrhythmia, chest pain, collapse.

At the onset of an anaphylactic episode, it is not possible to predict how severe it will become, how rapidly it will progress, and whether it will resolve promptly and completely or not.
Epinephrine should be administered as soon as possible once anaphylaxis is recognized or if impending anaphylaxis is suspected, even if patients do not meet diagnostic criteria

💡یه توضیح درباره آمپولهای اپینفرین(چون گاها ممکنه گیج شیم توی اون شرایط اورژانسی)، من دوتا آمپول اپی نفرین دیدم، یکیش کوچیک که عددش میشه 1mg/1ml و اون یکی که بزرگتره و میشه 1mg/10ml(یعنی یه جورایی رقیق شدهس و برای اینکه یادمون بمونه عدد ۱۰ رو که بزرگتره نشونه بذاریم).
مثلا توی CPR که IV میزنیم و بهتره رقیق شده باشه، میگه 1mg رو هر ۳تا۵ دقیقه بزنین یعنی یک آمپول بزرگ(10mg/ml) رو بزنیم.
توی انافیلاکسی چون IM میزنیم نیازی به رقیق کردن نیست، دوز اپی نفرین هم میشه(0.01mg/kg , max dose:0.5mg). پس مثلا بخوایم نیم میلی گرم بزنیم، باید یه آمپول کوچک یک میلی لیتری بیاریم و نصفش رو بزنیم(با سرنگ انسولین مثلا).
In summary, the 0.1 mg/mL solution is used for IV/IO administration in cardiac arrest to allow safe, controlled dosing, while the 1 mg/mL solution is used for IM administration in anaphylaxis to ensure rapid absorption and efficacy with a small injection volume.
🔹️Angioedema:
typically affects the skin and mucosal tissues of the face, lips, mouth, and throat, larynx, extremities, and genitalia, often in an asymmetric pattern. Angioedema can also affect the bowel wall and present as colicky abdominal pain. Episodes of isolated angioedema characteristically peak and then gradually resolve over the course of either hours (if mild) or three to five days (if severe).
