🔺Clues in History and P/E:
• most of cases are asymptomatic(or non-specific symptoms)
• Symptoms of anemia (fatigue, pica, restless leg syndrome, pagophagia)
• fingernail change, brittle skin
• Autoimmune gastritis or celiac disease
• Heavy menses or prior pregnancy
• GI bleeding or frequent blood donation
🔺lab:
• low RBC count
• mcv (nl or low), low mchc
• high rdw
• low retic count
• high plt (esp in severe or longstanding cases)
🔺PBS:
• hypochromia, poikilocytosis,microcytosis, anisocytosis and target cells.
🔺Diagnosis —
*any one of the following findings in the appropriate clinical sitting:
1️⃣ Serum ferritin <30 ng/mL. Some individuals may be diagnosed with iron deficiency at higher ferritin levels, especially if symptomatic or having a chronic inflammatory state.
2️⃣ Transferrin saturation <20 percent, mostly used in patients for whom the ferritin is thought to be unreliable due to an inflammatory state.; (TSAT= Serum Iron/TIBC)
3️⃣ Anemia that resolves upon iron administration.
☝🏻administration of iron to an individual with thalassemia will worsen the existing iron overload commonly seen in this condition. Thus, it may be prudent to obtain iron studies to confirm the diagnosis even in cases where iron deficiency is considered extremely likely.
4️⃣ Absence of stainable iron in the bone marrow (providing that adequate staining controls are performed).
🔺Tips:
1. Mentzer index: Calculated as MCV/RBC. A value >13 suggests IDA, while <13 favors thalassemia trait.
2. Rule of 3: Hb/RBC ratio < 3 suggests thalassemia trait, while a ratio ≈ 3 is more consistent with IDA.
3. Low RBC count is characteristic of iron deficiency and increased RBC count is characteristic of thalassemia. A typical RBC count for a patient with a hemoglobin of 9 g/dL is approximately 3 million/microL
4. All patients with microcytic anemia– should have iron studies. [serum iron, (TIBC)/transferrin, serum ferritin(iron stores), with calculated (TSAT)]; even when the diagnosis of thalassemia or ACD/AI is likely (such as in an individual with a known family history of thalassemia).
5. CRP and ESR do not distinguish between IDA and anemia of chronic disease, but their elevation should prompt caution in interpreting ferritin and iron studies.
6. Low MCV and low MCHC may be absent in early or mild iron deficiency, and can be masked by coexisting macrocytic processes or combined nutrient deficiencies.
❗️ACD/AI
Mild microcytosis(eg. 75-80) with low iron, low TIBC;
high-normal to high ferritin in the appropriate clinical context (eg, chronic inflammatory condition with normal MCV prior to its development) is consistent with ACD/AI.

❗️دلیل علمی: کاهش تولید ترانسفرین (TIBC) به عنوان واکنشگر فاز حاد منفی/ افزایش فریتین به عنوان واکنشگر فاز حاد مثبت در شرایط التهاب و بیماری مزمن.
اگر فرض کنیم آهن سرم «مسافران» (که در خیابان در حال جابهجاییاند)، ترانسفرین/TIBC: «تاکسیها» (ظرفیت ناوگان حملونقل برای سوار کردن مسافر)، فریتین: «هتلها» (محل ذخیره و استراحت مسافران) و درصد اشباع (Saturation):«شلوغی داخل تاکسیها» (چقدر از صندلیها پر شده است) باشد.
- در فقر آهن (Iron Deficiency): وضعیت شهر «قحطی مسافر» است. هتلها (فریتین) کاملاً خالی شدهاند. سازمان تاکسیرانی برای جبران، با ناامیدی تمام تاکسیهای موجود را به خیابان میفرستد (TIBC بالا)، اما چون مسافری نیست، اکثر تاکسیها خالی میچرخند (اشباع بسیار پایین).
- در آنمی التهاب/مزمن (ACD): ووضعیت شهر: «حکومت نظامی» در اینجا آهن وجود دارد، اما به خاطر شرایط جنگی (عفونت، سرطان یا بیماری خودایمنی)، بدن احساس خطر کرده و سیستم را قفل میکند (نقش هپسیدین).. مسافران هستند اما در هتلها حبس شدهاند تا آسیب نبینند (فریتین نرمال یا بالا). سازمان تاکسیرانی هم به خاطر شرایط امنیتی، تعداد تاکسیها را کم کرده و به پارکینگ برگردانده است (TIBC پایین). هم مسافر در خیابان کم است (آهن سرم پایین) و هم تعداد تاکسیها کم شده است (TIBC پایین).
