The cost of failure is very different in different projects.

Many things we do is where failure is cheap: non-life-important but cool hobby projects, admin panels where the process is slow and you can fix quickly, throwaway projects.

But some things we do are where the cost of failure is very high. An error in production may cost a lot of money or even human lives.
