📄 Abstract:
Aims: This systematic review examined whether meniscal centralization techniques—specifically posteromedial suture anchor (PMA) and posteromedial transtibial pull-out repair (PMP)—when added to conventional medial meniscus root repair, affect outcomes in patients with meniscal extrusion (MME).
Methods: A systematic search of PubMed, Embase, Scopus, and Web of Science identified studies using centralization techniques alongside root repair in cases of medial meniscus root tear with extrusion. Data on technique, outcomes, and meniscal extrusion changes were extracted.
Results: Eleven studies (totaling 474 patients) were included. Five studies used PMA, five used PMP, and one used both. Comparative studies found no benefit with PMP, while PMA showed significant advantages. In the PMA group, mean reductions in MME ranged from –1.2 mm to –0.2 mm. The PMP group showed inconsistent outcomes, some even showing increased extrusion (MD from –0.50 mm to +1.46 mm). Both techniques improved clinical scores, but the PMA technique consistently reduced extrusion.
Conclusion: Compared to isolated root repair, meniscal centralization procedures can significantly improve clinical outcomes and reduce meniscal extrusion—but effectiveness depends on the surgical technique, with PMA showing more consistent benefits.

📄 Abstract:
Intertrochanteric fractures account for nearly half of all hip fractures and are increasing with population aging. This systematic review and meta-analysis compared the newly designed proximal femoral bionic nail (PFBN) with the conventional PFNA and InterTAN systems. Eleven retrospective cohort studies were analyzed. PFBN demonstrated significantly shorter fracture healing time and earlier weight-bearing compared to PFNA and InterTAN, with no differences in complication rates, functional outcomes, or blood loss. These findings suggest that PFBN provides comparable or superior biomechanical and clinical performance, making it a promising alternative for intertrochanteric fracture fixation.

📄 Abstract:
Objectives: Total knee arthroplasty (TKA) success depends on precise soft tissue release and bone cuts to achieve balanced biomechanics. However, predicting the extent of medial soft tissue release (MSTR) and determining the appropriate distal femoral valgus cut angle (VCA) remain challenging. This study explored the relationship between radiographic parameters, MSTR, and VCA to identify predictive factors guiding intraoperative decision-making.

Methods: In a prospective cohort of 62 limbs (57 patients), preoperative lower-limb radiographs were analyzed. Univariate and multivariate regression models assessed correlations between radiographic indices, MSTR stage, and VCA.

Results: VCA showed strong positive correlations with medial hip offset (MHO) and femoral length (FL) (P < 0.001). Patients with shorter femurs and MHO > 4.35 cm required a distal femoral cut ≥ 6°, while those with longer femurs and MHO < 4.35 cm needed < 6° (SEN = 83%, SPE = 80%). Joint line congruency angle (JLCA), varus angle (VA), and lateral distal femoral angle (LDFA) correlated significantly with MSTR, with VA being the strongest predictor (SEN = 91.7%, SPE = 100%).

Conclusion: Increasing LDFA > 93.5°, JLCA > 7.5°, and VA > 19° increases the likelihood of extensive MSTR. MHO and FL are key predictive factors for determining the ideal distal femoral cut angle to optimize biomechanics and soft tissue balance in TKA.

📄 Abstract:
Objectives: This preliminary study compares two nerve transfer techniques for restoring elbow function in patients with C5–C6 brachial plexus injuries. The double nerve transfer (Oberlin II) involves fascicles from the ulnar and median nerves to the biceps and brachialis muscles, while the triple nerve transfer (Oberlin III) includes an additional transfer from the radial nerve to the brachioradialis muscle.
Methods: Nine patients with C5-C6 traumatic brachial plexus injuries were included and followed for five years after nerve transfer surgery. Four patients underwent the Oberlin III technique, while five received the Oberlin II procedure. Outcomes assessed included elbow and shoulder joint range of motion, hand grip strength, elbow flexion strength, elbow flexion velocity, and brachioradialis muscle bulk.
Results: All patients achieved an MRC grade of 4 in elbow flexion strength. Brachioradialis muscle bulk was recovered in the Oberlin III group. This group also showed higher elbow flexion velocity (P = 0.032). Furthermore, higher mean values were observed in the Oberlin III group for elbow flexion strength (in both supination and pronation) and QuickDASH scores; however, we did not detect statistically significant differences for these outcomes.
Conclusion: The Oberlin III technique was associated with recovery of brachioradialis muscle bulk and improved elbow flexion velocity. These preliminary findings suggest potential functional benefits, warranting further investigation in larger cohorts.

🎙 گفت‌و‌گو با دکتر سپهر متانت | روایت ساختار اجرایی کوتار: از تولد تا بلوغ

دکتر سپهر متانت، معاون اجرایی پیشین مرکز تحقیقات میان‌رشته‌ای کاربردی ارتوپدی (COTAR) هستن که از چهار سال قبل عضو کوتار شدن. از همون زمان، ایشون مدیریت امور اجرایی COTAR رو برعهده گرفتن و خیلی از ساختارهای اجرایی و برگزاری رویدادهای COTAR مدیون زحمات ایشونه.

دکتر متانت در آزمون دستیاری امسال موفق به قبولی در رشته ارتوپدی شدن و بعد از چهار سال فعالیت مؤثر در مرکز تحقیقات COTAR، برای رزیدنتی ارتوپدی جمع ما رو ترک کردن. به همین بهانه، فرصت رو غنیمت شمردیم و قبل از رفتن، باهاشون به گفت‌وگو نشستیم تا از تجربیاتی که طی این سال‌ها در COTAR به دست آوردن گپ بزنیم و داستان شکل‌گیری ساختار اجرایی مرکز تحقیقات COTAR رو از تولد تا بلوغ، مرور کنیم.

⚠️ پ.ن: ضبط این گفت‌و‌گو با چالش‌هایی مواجه شد که داشت ما رو به این فکر فرو می‌برد که منتشرش نکنیم، ولی دکتر نبیان (رئیس مرکز تحقیقات COTAR) یه نکته آموزنده برامون تعریف کردن که با شنیدنش قانع شدیم گفت‌و‌گو رو منتشر کنیم. این صحبت دکتر نبیان هم به ابتدای پادکست اضافه شده، حتما بشنوین :)
