📄 Abstract:
During the transportal technique of anterior cruciate ligament (ACL) reconstruction, tunnel outlet location can be varied depending on certain anatomical and technical characteristics. Therefore, we aimed to find out the acceptable zone of endobutton location by introducing several radiographic values. Postoperative lateral radiographs of 72 patients were assessed to measure the distances from the centre of the button to the posterior femoral cortex (D1) and to the most distal point of the lateral condyle (D2). Furthermore, based on the anteroposterior (AP) radiographs, the distances from the centre of the button to the lateral femoral cortex (D3) and from the centre of the button to the line connecting the most distal points of the medial and lateral femoral condyles (D4) were assessed. To measure the sensitivity and specificity of each radiographic value (D1, D2, D3 and D4), the area under the receiver operating characteristic curve was calculated. The alpha angle and femoral tunnel length values were considered as gold standards. Analyses showed that the mean values for D1, D2, D3 and D4 were 13.20 ± 0.54, 39.44 ± 0.31, 1.65 ± 0.15 and 42.66 ± 0.47 mm, respectively. The mean angle was found to be 38.6 ± 0.3°, and the mean femoral tunnel length was 38.6 ± 0.2 mm. Age was significantly related to D2 and the diameter of the femur in AP X-ray, while body mass index had a significant relation with D3 (p < 0.05). In this study, a new method was proposed to evaluate the accuracy of anatomical tunnel placement in ACL reconstruction surgery postoperatively. The statistical analysis of the measured variables showed that the mean ratios were 21.79 ± 0.87 for D1, 65.65 ± 0.63 for D2 and 51.90 ± 0.73 for D4. The results indicated that if the tunnel exit location and endobutton placement in the postoperative radiological images fall within the suggested areas, it can be meaningfully concluded that the tunnel is correctly positioned intraarticularly and the ligament reconstruction is anatomical.

📄 Abstract:
Advanced imaging techniques, such as C-arm fluoroscopy, O-arm, and CT navigation, are integral to achieving precision in orthopedic surgeries. However, these technologies also expose patients, surgeons, and operating room staff to varying levels of radiation. This systematic review and meta-analysis evaluate the radiation exposure (RE) associated with these imaging modalities and their impact on surgical outcomes. A comprehensive literature search was conducted following PRISMA guidelines, resulting in 2,725 identified articles. After removing duplicates and screening for eligibility, 24 studies were included in the analysis. Radiation exposure data, measured in milliSieverts (mSv) and milliGray (mGy), were standardized using conversion formulas. Quality assessments were performed using the Newcastle-Ottawa Scale (NOS) and ROB2 tools. Statistical analysis was conducted using random-effects models for comparing radiation exposure and fixed-effects models for secondary outcomes. The meta-analysis included 11 studies: 8 studies comparing C-arm and CT navigation, and 3 studies comparing C-arm and O-arm technologies. The analysis revealed that CT navigation is associated with significantly higher RE compared to C-arm (Standardized Mean Difference (SMD): 4.73, 95% Confidence Interval (CI): 2.44 to 7.03; p < 0.0001). In contrast, there was no significant difference in RE between O-arm and C-arm (SMD: 1.34, 95% CI: -0.17 to 2.85; p = 0.082). Secondary analyses showed no significant differences in surgery duration or hospitalization length between CT navigation and C-arm techniques. The results of this meta-analysis underscore the trade-offs between radiation exposure and surgical precision. While CT navigation significantly increases RE compared to C-arm fluoroscopy, it offers superior accuracy, particularly in critical precision surgeries such as spinal interventions. The lack of significant difference in RE between O-arm and C-arm technologies suggests that O-arm may provide a balanced approach, offering enhanced accuracy with radiation levels similar to C-arm. However, the significant heterogeneity among studies and inconsistent reporting of secondary outcomes indicate the need for further research. Future studies should focus on refining imaging techniques to optimize the balance between radiation safety and surgical accuracy. C-arm imaging generally results in lower radiation exposure compared to CT navigation, making it preferable for standard procedures where extreme precision is not as critical. However, CT navigation's superior accuracy justifies its use in precision surgeries despite the higher radiation exposure. O-arm technology, with its comparable RE to C-arm and enhanced accuracy, represents a beneficial option where available. Ongoing research should aim to optimize imaging techniques, balancing the need for radiation safety with the demands for surgical precision.

Entrapment neuropathies, marked by nerve compression at various anatomical sites, can be effectively managed using conservative approaches like injections. Dextrose 5 % water injection has emerged as a potential therapy by reducing inflammation and promoting tissue regeneration. We aimed to evaluate dextrose injection's efficacy in treating entrapment neuropathies in upper extremities. We systematically searched EMBASE, Scopus, Web of Science, and PubMed. Our eligibility criteria included participants aged 18 and older who had peripheral upper limb nerve entrapment from non-metabolic and non-traumatic sources. These participants were treated with dextrose injection compared to those receiving other injectables, such as corticosteroids and non-corticosteroid medications. The primary outcome was pain, with secondary outcomes including function, ultrasonographic, and electrodiagnostic findings. The quality of the clinical trials was assessed using Cochrane tools. Random-effect model was employed for meta-analysis. Thirteen studies, involving 754 patients, were included, with only two showing serious bias risk. Initial findings indicate significant pain relief with dextrose injection within a short time (≤4 weeks) compared to normal saline (MD: −1.30, 95%CI: −2.12; −0.47). Dextrose also demonstrated a significant pain decrease compared to corticosteroids (MD: −0.81, 95 % CI: −1.40; −0.21) with low heterogeneity (I2 = 9 %, P = 0.36), and significantly improved function (MD = −0.46, 95 % CI: −0.76; −0.16) with low heterogeneity (I2 = 17 %, P = 0.31) in mid-term (one to six months). This meta-analysis suggests dextrose injection as an effective therapy for managing pain and restoring function in entrapment neuropathies. However, further high-quality studies are needed to confirm these findings and establish optimal dosing regimens and injection protocols. Healthcare providers should consider integrating dextrose injection into their treatment strategies for patients with entrapment neuropathies.

📄 Abstract:
The foot and ankle complex is crucial in stability, propulsion, and musculoskeletal interactions. Optimizing orthotic interventions in this anatomical region is essential to improve motor outcomes. This systematic review investigates the impact of foot orthoses (FOs) with sensorimotor bars on gait, postural control, and muscle activity in healthy individuals and those with musculoskeletal disorders. We conducted an electronic search in January 2024 using the PubMed, Web of Science, and Scopus databases. The article identification, screening, and selection followed the PISMA (the preferred reporting items for systematic reviews and meta-analysis) guidelines. We conducted the quality assessment using the PEDro (the physiotherapy evidence database) checklist. Data extraction and synthesis were performed using the Cochrane handbook for systematic review of interventions. FOs with sensorimotor bars can increase foot external rotation, dorsiflexion, and abduction while decreasing foot eversion. Additionally, they can reduce anteroposterior displacement of the hip, shoulder, and head. Furthermore, significant improvements have been observed in spatiotemporal parameters such as walking speed, stride length, stance time, swing time, and kinetic parameters such as plantar pressure. These orthoses can also impact clinical tasks and decrease center-of-pressure movements. Sensorimotor bars have demonstrated beneficial effects on gait (kinematic, kinetic, and spatial-temporal measures), balance control (clinical performance and center of pressure displacement measures), and muscle activity recording.

The medial collateral ligament is the most common knee ligament to be injured. Although mostly treated nonoperatively, a distal tear of the ligament is a clear indication for surgery. High failure rates with unreliable fixation methods have largely made knee surgeons reluctant to perform ligament repairs. Here, we propose a repair technique that uses the periosteal cambium layer to achieve surface contact healing, instead of point contact fixation, and combine mechanical fixation with regenerative enhancement.

📄 Abstract:
Orthopedics is one of the specialized fields of medicine and its residency has always been of interest. This systematic review and meta-analysis aim to evaluate the prevalence of depression and anxiety among orthopedic residents. Digital databases such as PubMed, Scopus and Web of Science were systematically searched until July 27, 2024. The analysis included a total of 10 studies for anxiety (1,758 cases, 374 anxiety cases) and depression (2,389 cases, 381 depression cases). The pooled prevalence of anxiety was 31.6% [95% CI: 18.04–45.26%] with significant publication bias (Egger’s test p = 0.0257). After removing a potential outlier, the prevalence of anxiety was recalculated as 25% [95% CI: 16.61–34.01%]. The pooled prevalence of depression was 22.2% [95% CI: 12.08–32.51%] with significant publication bias (Egger’s test p = 0.0434). After removing a potential outlier, the prevalence of depression was recalculated as 18.6% [95% CI: 10.27–26.92%], showing no significant publication bias (Egger’s test p = 0.1229). Depression and anxiety are highly prevalent among orthopedic residents, with significant levels of psychological distress affecting nearly one-third of this population.
