📄 Abstract:
The interobserver and intraobserver reliability of various tibial plateau fracture (TPF) classifications has been examined in recent literature using radiography, computed tomography, and magnetic resonance imaging. The question remains as to which classification system provides the highest reliability. In this systematic review, we are going to evaluate the overall interobserver and intraobserver reliability of various TPF classifications in different imaging modalities.
We conducted a systematic review following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. In February 2023, predefined terms were used for database search (Embase, PubMed, Scopus, Cochrane, and Web of Science). Meta-analysis of intrarater and inter-rater kappa coefficients was performed for each of the classifications in each modality. Thirty-four studies were included in this review. Schatzker's classification was more frequently used than others. It had a better intrarater kappa coefficient than the Hohl and Moore and Arbeitsgemeinschaft für Osteosynthesefragen/Orthopedic Trauma Association (AO/OTA) classifications in radiography (κ = 0.72, 95% confidence interval [CI] = 0.67-0.76, p < 0.01). The Schatzker and AO/OTA classifications had similar inter-rater reliability in the radiography modality (κ = 0.53, 95% CI = 0.51-0.54, p < 0.01; κ = 0.53, 95% CI = 0.5-0.55, p < 0.01; respectively). In 3-dimensional computed tomography, the Luo classification system showed the highest intrarater (κ = 0.85, 95% CI = 0.35-0.66) and inter-rater (κ = 0.77, 95% CI = 0.73-0.81) kappa coefficients. Three-column classification proposed by Luo et al. was able to reach the highest degree and was the only classification with near-excellent inter-rater reliability.

📄 Abstract: An increase in knee joint loading exacerbates the symptoms of knee joint osteoarthritis (OA). One of the conservative treatments used for subjects with knee OA is knee valgus orthosis, which is used to decrease the loads on knee joints. The aim of this study was to determine the effects of a new design of knee orthosis on joint contact force and muscle force in subjects with knee OA. Ten subjects with knee OA were recruited for this study. A motion analysis system with seven high-speed cameras and a Kistler force plate were used to record the motion of the subjects in walking and the forces applied on the leg. We used OpenSIM software to determine the knee joint contact force during walking, with and without the orthosis. The knee orthosis decreased the peaks of the vertical component of knee joint contact forces (p < 0.05). Moreover, it did not influence walking speed. The use of the orthosis decreased the extension moment of the knee joint and the peaks of the forces produced by the muscles surrounding the knee joint. The use of this orthosis decreased the knee joint contact forces. This suggests that the orthosis could be used to alleviate the symptoms of knee OA. Orthosis can be incorporated into the clinical management of subjects with knee OA by medical practitioners. The study demonstrated that the new design of knee orthosis effectively reduces the knee joint contact and muscle forces in subjects with knee osteoarthritis without affecting walking speed. These suggest that orthosis could be a valuable conservative treatment option to alleviate symptoms and manage knee osteoarthritis in clinical practice.

📄 Abstract: This study aimed to compare the findings of plain radiography and computed tomography (CT) of foot and ankle in patients submitted to the emergency department with high-energy foot and ankle trauma, to demonstrate if missing fractures on plain radiographs will significantly alter the treatment plan considered for each patient based on the findings of each imaging modality. We retrospectively observed standard radiological foot and ankle x-rays and CT scans in patients who presented to our center from April 2019 to June 2020 with a history of foot and ankle trauma with either loss of consciousness, a history of high-energy trauma, or clinical presentation disproportionate to plain radiographic findings. We investigated the number of fractures of each bone detected on plain radiographs and CT scans and the treatments based on each modality's findings in patients admitted to our center. Sixty-five out of 163 (39.87%) included in our study had at least one missed fracture on plain radiography that was detected on CT. Thirty-one (19%) patients had normal radiography despite actually having fractures. In 38 (23.31%) patients CT changed the treatment plan decided by our surgeons (P < 0.001). The two imaging modalities had a moderate agreement for detecting foot and ankle fractures overall (κ = 0.432). The failure to detect fractures in patients with high-energy trauma can significantly impact treatment effectiveness. Integrating CT scans into the diagnostic process can lead to changes in treatment planning and ultimately improve patient outcomes.

📄 Abstract: Ankle instability results in sensory and motor impairments. Typically, health professionals recommend conservative treatment as the initial approach for individuals with this condition. One such treatment option is Electrical Stimulation (ES). This systematic review assesses the effects of ES techniques on postural control measures in cases of ankle instability. We systematically searched five electronic databases: ClinicalTrials.gov, PubMed, Scopus, SPORTDiscus, and Web of Science. To evaluate the quality of the included articles, we utilized the PEDro checklist. We extracted data on population, intervention, and outcomes and synthesized them narratively. ES decreased the time needed to stabilize the center of pressure, velocity, displacement, and area, thereby enhancing the performance of clinical tests. While postural stability indexes remained unaffected, the gait inversion angle increased with electrical stimulation. These results suggest that ES interventions are crucial in enhancing postural control in subjects with ankle instability compared to coordination exercises therapy alone. Stochastic resonance reduced A/P and M/L TTS, COP velocity, COP displacement, and COP area, resulting in enhanced postural control.

📄 Abstract: Over the past 3 decades numerous studies have reported an association between systemic lupus erythematosus (SLE) and thyroid cancers. However, there has been no scientometric analysis in this area of research. To perform a comprehensive scientometric analysis of the global literature published on the association between SLE and thyroid cancers. The data on publications within thyroid cancers in SLE patients were retrieved from the Scopus database using a defined search strategy from its first publication in 1964 to 2023. To conduct a collaboration mapping analysis among keywords, authors, journals, and territories, VOSviewer was utilized. Our final research resulted in 246 scientific publications with 8072 citations, which were published in 198 journals affiliated to 48 countries. A global upward trend has been observed in the last 20 years, with the highest number of publications in the year 2022 (n = 28; 11.4%). The United States led the global productivity ranking with 74 publications (30.1%), followed by China with 25 publications (10.2%). The most popular journals in this field were "Arthritis Research and Therapy" and "Frontiers in Endocrinology," while the most co-cited journal was "Autoimmunity Reviews." The top 3 most prolific authors were Bernatsky, S., Clarke, A.E., and Ramsey-Goldman, R with 9 publications each. This first scientometric study comprehensively offered an overview of the status of thyroid cancers in SLE patients, assessing scholarly productivity in this domain over a period of 50 years.

📄 Abstract: Proximal femoral varus osteotomy (FVO) is one of the most used treatment methods with acceptable outcomes for Legg–Calvé–Perthes disease (LCPD). We aimed to investigate the influence of age at disease onset and the Lateral Pillar classification on clinical and radiological outcomes of FVO surgery LCPD patients between 6–12 years of age. Proximal FVO surgery in the early fragmentation phase of LCPD patients led to acceptable clinical and radiographic outcomes in a 3-year follow-up, regardless of preoperative age and Herring type. Fifty patients with LCPD (Herring groups B, B/C, and C) who underwent FVO were retrospectively reviewed. We evaluated radiological [center-edge angle, extrusion index, epiphyseal index, acetabular index, articulo-trochanteric distance (ATD)] and clinical [hip abduction range of motion (ROM), Trendelenburg sign, pain, and Harris hip score (HHS)] outcomes with a follow-up of 37.3 ± 10.5 months (range: 24–180 months). Finally, the overall treatment outcome was assessed using the Stulberg classification. The ROC curve analysis did not reveal any significant relationship between age and clinical or radiological outcomes, and there was no predictable age cut-off for surgical outcomes (p = 0.13). No significant difference was found in Stulberg classification at the follow-up between patients with type B, B/C, and C of the lateral pillar (p > 0.05). Our results demonstrated that open-wedge proximal FVO surgery in the early fragmentation phase of LCPD patients led to acceptable clinical and radiographic outcomes in a 3-year follow-up. Each sample of our study was very small and a lot of variables were measured, making this result not adequately strong enough to draw a robust conclusion. However, FVO surgery remains a possible suggestion for patients in the early fragmentation phase, and age and lateral pillar type may not be limiting factors.

📄 Abstract: The primary objective of revision total knee surgery is to achieve solid bone fixation. Generally, this could be accomplished using sleeves and long stems, which require substantial remaining bone stock and may increase the risk of stem tip pain. An alternative approach involves the use of customized diaphyseal cones, which can preserve the integrity of the bone canal. This study evaluates the impact of employing femoral diaphyseal cones with various stem lengths on stress distribution and relative motion. CT scan data from five patients were used to generate the 3D model of the femur, cement, customized stems, and cones, along with assigning patient-specific material for each candidate’s femur. Three different stem lengths, both with and without the customized cone, were assessed under three gait loading conditions to compare the resulting Von Mises stress distribution and relative motion. Analysis indicated that the use of customized femoral cones moderately increases stress distribution values up to 30 % while significantly reducing relative motion at the femoral canal-cone interface by nearly 60 %. The presence of the cone did not significantly alter relative motion with varying stem lengths, although stem length variation without a cone substantially affected these values. Incorporating cones alongside stems enhances metaphyseal fixation, reduces stress shielding, potentially allowing for the use of shorter stems. Furthermore, cones promote osseointegration by minimizing relative motion, ultimately improving prosthetic stability.

📄 Abstract: Ankle orthoses are one of the interventions used to prevent first and repeated lateral ankle sprains. We conducted this systematic review of the literature to investigate the effect of ankle orthoses on postural control in individuals with acute, chronic, or functional ankle sprains. Three electronic databases (Scopus, PubMed, and Web of Science) were searched for experimental studies using predefined keywords. We used the PRISMA method to select articles. Initially, the articles were examined for inclusion criteria and then assessed for their quality based on the physiotherapy evidence database scale. In the initial search, we retrieved 156 articles. After following the steps of the PRISMA flowchart and reviewing the entire text, only 14 reports met the inclusion criteria of this study. We extracted data from population, intervention, and outcome measures. Overall, ankle orthoses were effective in improving the static postural control of patients with lateral ankle sprains; however, the dynamic postural control results were unclear. Since most people who experience lateral ankle sprains have both functional and mechanical instability, ankle orthoses may affect both types of instability and thereby improve postural control.

📄 Abstract: Fractures in the lower extremities are sometimes accompanied by severe damage, so definitive treatments with extensive surgical exposure are impractical. Extensive traumatization and swelling of surrounding tissue often dictate the use of external fixators as a temporary treatment for immediate stabilization of compound fractures. External fixation offers a variety of possible spatial configurations. Adequate fracture stabilization demands a good understanding of the stiffness, strength, sustainability, and tilting of external fixators of different frames. An improper frame can result in nonunion, malunion, delayed healing, and bone re-fracture, some of which are indications for revision surgery. This study employs a numerical approach to assess the stiffness, stress distribution, pin loosening, and interfragmentary displacement in different configurations of external fixators applied to a comminuted fracture in the diaphyseal region of the tibia. Unilateral fixators with single and double rods, bilateral, biplanar, and triangular frames with and without end cross-links are different configurations examined in-silico. Results show that the triangular frame with cross-links exhibits the stiffest, strongest, and most sustainable construct in axial, bending, and torsion modes. Except for the torsion mode, adding the end cross-links does little to increase the stiffness and strength of biplanar and triangular frames. Moreover, doubling the rod considerably improves construct stiffness and strength under axial compression load while appearing to be less superior in torsional stiffness and pin loosening. Furthermore, the bilateral frame demonstrates the most uniform displacement across the fracture gap. The results of this study could be used for preoperative planning of diaphyseal fracture management with external fixators.
