🇺🇸⚡️ — A U.S. Army investigation found multiple systemic failures behind the January 2025 collision between a Black Hawk helicopter and an American Airlines jet near Washington's Reagan National Airport, which killed 67 people, according to WaPo.
The report cited faulty Black Hawk altimeters, inadequate pilot training, failure to track near misses and poor coordination in congested airspace. The helicopter's altimeter showed 200 feet when it was actually flying at 278 feet, while night-vision goggles may have obscured the approaching jet and caused the crew to focus on another aircraft.
The findings largely support an earlier NTSB investigation that primarily blamed the FAA for allowing military and civilian flight routes to pass within 75 feet of each other. Investigators also raised concerns about air traffic control instructions and the military's limited use of aircraft tracking systems.
