اولا در این سوال طراح واحدهای شمارش رتیکولوسیت، پلاکت، و نوتروفیل را نکرده و با توجه به رویکرد طراحی سوالات که از کوچکترین تغییر و بازی با واحد و حروف و اعداد هم برای درست و غلط بودن گزینه ها استفاده می کنند، این سوال غلط طراحی شده زیرا مشخص نیست منظور از تعداد در گزینه های الف و ب و ج در چه حجمی است؟؟؟

و از طرفی عدد ۵۰۰۰۰ برای معیار رتیکولوسیت در منبع نیامده و معیار ۲۰۰۰۰ هست، در موارد شمارش دستگاهی هم معیار ۶۰۰۰۰ ذکر شده است.( که اگر منظور این بود باید در گزینه اشاره میشد به روش دستگاهی)…
بنابراین این سوال یا باید حذف شود، یا اینکه هردو گزینه ج و د صحیح در نظر گرفته شود( هرچند گزینه ج صحیح تر است و تنها دلیل طراح برای درنظر گرفتن گزینه (د) احتمالا عدم اشاره هنری به هموگلوبین در معیارهای آپلاستیک شدید بوده است، در حالی که عدد هموگلوبین ۷ قطعا دلالت بر شدت آنمی دارد)

Aplastic Anemia AA usually refers to pancytopenia associated with a severe reduction in the amount of hematopoietic tissue that results in deficient production of blood cells in the absence of a bone marrow infiltrative process or increased reticulin (Clucas, 2019). Usually two of the following three blood parameters are needed to diagnose AA: Hgb less than 100 g/L, granulocyte count less than 1.5 × 109/L, and platelet count less than 50 × 109/L. The marrow, although hypocellular, may have patchy areas of normocellularity, or even hypercellularity. The diagnosis of severe AA is made in pancytopenic patients when at least two of the following three peripheral blood values— granulocyte count less than 0.5 × 109/L, platelet count less than 20 × 109/L, or reticulocyte count less than 20 × 109/L, or 1% (corrected for Hct)—are present and the bone marrow is less than 25% cellular. With the pervasive use of automated reticulocyte counts, the threshold for severe AA stands at 60 × 109/L using automated counts (Dezern, 2019). The term very severe AA is used when the granulocyte count is less than 0.2 × 109/L (Clucas, 2019). Otherwise, the AA would be classified as moderate or nonsevere. AA is usually classified as inherited (see later discussion) or acquired. The incidence of acquired AA is 2 per million per year based on retrospective studies in Europe and North America, and is about 2 to 3 times higher in East Asia, with the highest being in Japan and estimated at 14 per million per year (Dezern, 2019). There is a bimodal age of peak presentation, with the first occurring in patients aged 15 to 25 years and the second in patients over 60 years old.
